SPECIAL MESSAGE TO INCOMING FRESHMEN:
Any service projects don't "count" toward fulfulling your yearly Christian Community Service requirement until you have submitted both the proposal AND evaluation forms for each project.
Your proposal should be submitted in person, by mail, via fax, or scanned/emailed) before you begin work on your project.
Your evaluation form must be submitted after the completion of work at your site. Be sure to get your evaluation form signed by your supervisor on the last day of work -- sometimes, it's tough to track down former supervisors after the fact!
Your service reflection will not be due until 4th quarter (date TBA)

Service Proposal: (to be completed at the beginning of each project)

This form must be completed by the Immaculate Heart student prior to beginning a new service project. After the form has been completed, the student should bring her form to the IHHS Activities Office, and submit it to the Director of Community Service (Ms. Buckowski). If there is a problem with the proposal, students will be notified via campus email within two days. Otherwise, students may consider their proposal approved, and they may begin service for credit.

This form must be completed by the volunteer site supervisor (not a parent or relative) upon completion of service. After the form has been completed, the supervisor may choose to submit the paperwork him/herself, or return the completed form to the student for final submission.
Paperwork may be submitted in any of the following ways:
- Return the completed form to the student, who should then deliver the form to the IHHS Activities Office.

After completing her required service hours for the year, the student must submit a written reflection on one particular service project, OR a collective reflection on all hours served that year. A creative/artistic component is encouraged, but not required.
Reflections must be submitted by the week after the CCS deadline: (date TBA) Freshmen should submit their reflections to their Theology teacher (Ms. Wong or Ms. James).
Sophomores, Juniors and Seniors should submit their reflections to the Director of Community Service (Ms. Buckowski).
